It’s time to start thinking about your move to adult health care services, but where to start?
This stage is about introducing you and your parents to the idea of transition. During this time, you should start to learn more about your condition and begin to start taking a little more responsibility for your health care. During this stage, you might begin to pay more attention to your health care routines. You could also consider the questions you would like to ask at your hospital or clinic visits. Your parent(s)/guardian(s) will be of support to you during this stage.
